We are “Out of Network” for all dental insurance. We do not accept any insurance payment in our office.
Our office does not participate in Medicare or Medicaid.
As a courtesy, we are happy to print your insurance claim form for you to send to your dental insurance company so they can reimburse you directly. We recommend you make a copy of the claim form. If the procedure requires images or narrative description, we will email the information to you within 2 business days.
Please complete this Insurance Inquiry form and bring it to your appointment so we have pertinent information for filing.
If you have Primary and Secondary dental insurance, we will give you both claim forms to file. Once you receive your explanation of benefits from your primary insurance, you will need to attach that to the secondary insurance claim form and submit for reimbursement. Please note: Secondary insurance will not reimburse you until they know what was paid by Primary insurance.
If you have questions about your reimbursement from your insurance company, please contact your insurance company. If your insurance company requires additional information, please fax/email/mail a copy of your explanation of benefits to me with a note letting me know.
We use the same Dental Codes as every other Dental Practice. If you have out-of-network benefits, you should expect some percentage of reimbursement for most procedures.
Once you come in for your thorough exam and we have formulated a treatment plan, we can print a pre-treatment estimate to your insurance company for you to file so you know in advance what they will reimburse. Please note: Insurance companies typically take 4-6 weeks to respond.
